An employee was driving a motorized buggy down an aisleway in the assembly area of a plant along with a co-worker. The employee braked to turn a corner when his foot slipped off the brake and on to the accelerator pedal. The motorized buggy turned the corner and went right into a mist collector. The employee was thrown forward in the buggy and suffered multiple fractures to his left foot that required surgery.

The injured employee and another employee were lining up a W access adapter/base plate. Three of the four bearing pins were located/lined up. The fourth pin was then located/lined up and the base dropped, pinching the injured employee's thumb. The employee pulled his thumb away from the base resulting in amputation of soft tissue.

An employee was troubleshooting the CNC line glue spreader and was attempting to push and pull a board through the spreader. When the employee pulled the wood through, the wood came down and crushed his finger, which was on a guide, between the guide and wood. His right middle fingertip was amputated.

An electrician at the Ford Motor plant was driving a scissor lift. He stopped the lift to begin work on an electrical panel. When he stopped the lift it rocked, causing his hand to get caught between the lift basket and the electrical panel. His left pinky finger was crushed resulting in the amputation of the fingertip.
